Do I need a prescription?
No. Perform is a food supplement and is available without a prescription.
How do I take it?
Take four capsules a day with food: two in the morning and two in the evening. Do not exceed the stated dose.
How long does one tub last?
A tub contains 120 capsules, which is one month at four capsules a day.

Can I cancel or pause my subscription?
Yes. There's no minimum term. Email hello@supplementsbyhazel.com or call 0117 462 3935 before your next payment date and we'll cancel, pause or change it for you.
Can I take it with medication?
If you take any medication or have a medical condition, check with your GP or pharmacist before starting a new supplement.
Is it suitable for vegetarians?
The capsule shell is plant-based (hydroxypropyl methylcellulose) and the vitamin D3 is from a vegan source.
Where is it made?
Perform is made in the UK for Supplements by Hazel Limited, Bristol.

- Dose

- Vitamin B6
- Long-term intake of this amount of vitamin B6 may lead to mild tingling and numbness.
